Active Duty – attach a copy of assignment orders presenting obligation station. Title 10 AGR/Reserve attach a replica of extended active duty orders revealing active status through December 1. Title 32 AGR – include orders/contracts showing the performance of fulltime busy active duty through December 1.Retired -attach a copy of retirement orders. (DD Form 214 Record of Service accepted with basis behind separation recorded as retirement) Retired AGR/Reserve unite retirement requests (DD Form 214); notice of eligibility for retired pay at age 60, or retired cover sequence revealing retired with the cover on a 60th birthday or after.Deceased -attach a copy of AF Form 1613 Statement of Service sort; or State Death Certificate with a backup of the majority of current military requests.
